Upon enrollment each semester, all students are required to accept the 糖心logo入口 (糖心logo入口) Student Financial Responsibility Agreement. The agreement outlines the financial terms and conditions associated with your registration. When registering for classes, you assume financial responsibility and accept the terms of this agreement. This agreement will pop up as a required action item before you can proceed with registration each semester. Payment of Account/ Promise to Pay

I understand that when I register for any class or receive any service from 糖心logo入口 I accept full responsibility to pay all tuition, fees and other associated costs assessed as a result of my registration and/or receipt of services. I further understand and agree that my registration and acceptance of these terms constitutes a legal binding contract with 糖心logo入口. I promise to pay all
assessed tuition, fees and other associated costs for this semester by the published or assigned payment due date, whether or not a bill has been received.

If I decide to not attend, it is my responsibility to drop my classes as 糖心logo入口 will not automatically cancel my registration. I understand and agree that if I withdraw or drop a class, I will be responsible for paying all or a portion of charges on my account in accordance with the published refund policy. I have read the terms and conditions of the published Drop Refund policy and understand those terms are incorporated herein by reference. I further understand that my failure to attend class or receive a bill does not absolve me of my financial responsibility as described above.

Financial Aid

I understand aid described as 鈥渁uthorized鈥 on my Financial Aid Award does not represent actual or guaranteed payment but is an estimate of the aid I may receive if I meet all requirements stipulated by that specific aid program. I authorize using any federal Title IV student aid credit balance to pay all educationally-related charges that may be assessed to my account, including books, fees, fines, and other incidental expenses. Any remaining credit balance after all institutional charges are paid will be refunded to me to use for other educationally related expenses.

I understand my Financial Aid Award is contingent upon my continued enrollment and attendance in each class upon which my financial aid eligibility was calculated. If I drop any class before completion, I understand my financial aid eligibility may decrease and some or all of the financial aid awarded to me may be revoked.

If some or all of my financial aid is revoked because I dropped or failed to attend class, I agree to repay all revoked, disbursed aid which may have already been paid to me through the refund process.

Delinquent Accounts

Financial Hold

I understand and agree that if I fail to make full financial payment and/or arrangement (financial aid or deferred payment plan) of all charges on my student account by the scheduled due date, 糖心logo入口 will place a financial hold on my student account, preventing me from registering for future classes, or receiving my diploma.

Late Payment Charge

I understand and agree that if I fail to make full financial payment and/or arrangement (financial aid or deferred payment plan) of all charges on my student account by the scheduled due date, I will be assessed a late fee of $10.00 per month with a maximum of $30.00 per semester.

Collection Agency Fees

I understand and agree that if I fail to make full financial payment and/or arrangement (financial aid or deferred payment plan) of all charges on my student account by the scheduled due date, 糖心logo入口 may refer my delinquent account to a collection agency. I further understand that if 糖心logo入口 refers my student account balance to a third party for collection, a collection fee may be assessed and will be due in full at the time of the referral to the third party. If my account is referred to an outside collection agency, I understand that I will be responsible to pay up to an additional 25% of my total past due balance in collections fees. I will also be responsible for all other fees, including reasonable attorneys鈥 fees and associated costs incurred, if my account is brought to court in either Rock Island County, Illinois or Henry County, IL. Finally, I understand that my delinquent account may be reported to one or more of the national credit bureaus.

Communication

Method of Communication

I understand and agree that 糖心logo入口 uses academic email as an official method of communication with me, and that therefore I am responsible for reading the emails I receive from 糖心logo入口 on a timely basis.

Contact

I authorize 糖心logo入口 and its agents and contractors to contact me at my current and any future cell phone number(s), email address(es) or wireless device(s) regarding my delinquent student account(s)/loan(s), any other debt I owe to 糖心logo入口, or to receive general information from 糖心logo入口. I authorize 糖心logo入口 and its agents and contractors to use automated telephone dialing equipment,
artificial or pre-recorded voice messages, text messages, personal calls, and emails, in their efforts to contact me. Furthermore, I understand that I may withdraw my consent to call my cell phone by submitting my request in writing to 糖心logo入口 Bursar鈥檚 Office or in writing to the applicable contractor or agent contacting me on behalf of 糖心logo入口.

Updating Contact Information

I understand and agree that I am responsible for keeping 糖心logo入口 records up to date with my current physical address(es), email address(es), and phone number(s) by filling out the Student Change of Address Form. The linked form is incorporated herein by reference. Upon leaving 糖心logo入口 for any reason, it is my responsibility to provide 糖心logo入口 with updated contact information for purposes of continued communication regarding any amounts that remain due and owing to 糖心logo入口.

IRS Form 1098-T

I agree to provide my Social Security number (SSN) or taxpayer identification number (TIN) to 糖心logo入口 upon request as required by Internal Revenue Service (IRS) regulations for Form 1098-T reporting purposes. If I fail to provide my SSN or TIN to 糖心logo入口, I agree to pay any and all IRS fines assessed as a result of my missing SSN/TIN. I consent to receive my annual IRS Form 1098-T, Tuition Statement, electronically from 糖心logo入口. I understand that a paper copy of the 1098-T can be requested at any time from the Bursar鈥檚 Office.

Privacy Rights & Responsibilities

I understand that 糖心logo入口 is bound by the Family Educational Rights and Privacy Act (FERPA) which prohibits 糖心logo入口 from releasing any information from my education record without my written permission. For more information, please contact Enrollment Services via email registrar@bhc.edu.
